The Petitioner, Huntingdon County Children and Youth Services Agency, hereinafter "the Agency," is the legal and physical custodian of the above-named child. The Agency was awarded legal and physical custody of R.P by Court Order (EPC) dated August 11, 2016. 2. The Respondent is C.P., hereinafter "the Natural Mother," whose last known address is 231 Silverford Hights, Mount Union, PA 17066. The Natural Mother is age 29 (DOB: 09/24/1987), and is a Caucasian female whose religious affiliation is unknown. 3. R.P. was declared dependent on September 22, 2016. 4. The Natural Father of the above-named child is C.P. whose last known address is 16909 Croghan Pike, Shirleysburg, PA 17260. 5. The Natural Father and the Natural Mother of this child were married to each other, but are now unmarried. 6. R.P has been in placement since August 11, 2016, based upon an Emergency Protective Custody Order entered by this Honorable Court. 7. The Natural Mother has failed to make progress regarding services that are provided to the child. 8. The child has been in placement in excess of twelve (12) months and the Natural Mother has made no progress in remedying the issues and concerns which led to the child's placement in August, 2016. 9. The Natural Mother appears incapable of providing proper care and control for the child. 10. The Agency believes and therefore avers that termination of parental rights and adoption of R.P. would best serve the needs of the child. 11. Petitioner Huntingdon County Children and Youth Services Agency will assume custody of the above-named child until such time as the child is adopted. 12. At the time of the filing of this Petition an adoption is contemplated by the paternal grandmother, B.W. 13.
